Artifact: crisis.blocklist_advisor_signed

Status: CLINICAL GATE MET 2026-09-21. Mic (Matt Miclette) signed off CRISIS_PROTOCOL.md v1.5 by email. That is version-specific clinical authorization of the protocol. Go-live remains conditional on LastVet’s remaining checklist (counsel, item 5 re-fire, signed version committed/deployed, Ryan flip GO).
Gate: Clinical advisor-of-record review of CRISIS_PROTOCOL clinical content (Section 2 handoff, Section 3 blocklist categories, Section 4 triggers, Section 5 reopen).
Sign-off copy: CRISIS_PROTOCOL.md v1.5 (2026-09-21). The Veterans Crisis Line response in Section 2 is unchanged from the 16 September review copy. Effect on product: Does not authorize LASTVET_CRISIS_LIVE_ENABLED=true. Live flip still requires counsel clear, item 5 on-device re-fire, v1.5 committed so signed == live, and a deliberate logged flip. Clinical sign-off is the clinical half of Meridian m1_eligible; PHI hosting + Arc counsel still apply separately.

Scope of Mic’s sign-off (important)

Mic’s confirmation is clinical-content endorsement (what the protocol says / must not say / how handoff+reopen behave). It is not validation of the deployed system.

His written confirmation (“Ryan, yes confirmed on all fronts.”, 2026-09-16) replied to the email enumerating the then-current blocklist + reopen. A follow-up scoped that endorsement and added before-go-live conditions, including four additional prohibited categories (functional equivalence, not exact phrases only) and a validation checklist.

Sign-off of record (clinical content):

Piece Date Form
Substantive review + four operational amendments 2026-08-18 Written review → protocol v1.1
Phone confirmation AS-IS on then-stated content 2026-09-16 Verbal
Written confirmation on enumerated blocklist + reopen 2026-09-16 Email: “Ryan, yes confirmed on all fronts.”
Scoped follow-up: clinical-only + go-live conditions 2026-09-16 Email follow-up (conditions below)
Version-specific clinical sign-off on v1.5 2026-09-21 Email (clinical gate MET). Not a live-flag authorization.

Advisor of record: Mic (Matt Miclette), matthew@neuroflow.com

Mic’s four additional prohibited CATEGORIES (must be in protocol before re-flip)

Defined as categories (paraphrases / functional equivalents count, not only exact phrases):

  1. Safety-commitment requests — e.g. “can you promise not to hurt yourself”
  2. Means / location / presence questions — location, access to weapons/means, whether someone else is present
  3. Risk determinations — e.g. “you are at high risk” / “you are not in immediate danger”
  4. Active-monitoring / intervention implications — e.g. “stay with me,” “I'm contacting someone,” “I'll make sure you're safe”

Protocol version tracking these: see CRISIS_PROTOCOL.md Section 3 (v1.3+) and iOS/Meridian blocklist graders.

Pre-go-live validation checklist (Mic) — required before LASTVET_CRISIS_LIVE_ENABLED=true again

Track in last.vet-api/docs/runbooks/gate-1/CRISIS_PRE_GO_LIVE_VALIDATION.md.

  • [ ] B-028 roleplay / multi-turn failure corrected + regression passing
  • [ ] Trigger reliably stops the prior task + shows approved Section 2 intact, no model-generated additions
  • [ ] Prohibition controls cover functionally equivalent language (phrasing variations), not just exact phrases
  • [ ] Testing includes direct / passive / veiled / contextual / quoted / hypothetical / multi-turn language
  • [ ] Dismissal / reopen / re-trigger across supported devices
  • [ ] Call / text / chat / TTY / accessibility / overseas pathways work
  • [ ] Privacy-preserving failure-monitoring + incident-investigation method
  • [ ] Counsel review (no-notification, post-dismissal, duty, retention, legal/regulatory)
  • [x] Consolidated dated protocol version (v1.5) signed by Mic against that version (email 2026-09-21). Clinical only.

Ops status (2026-09-16 evening)

  • Crisis live flag: restored to false (stub) on R940 staging + prod after Mic’s scoped follow-up — live Section 2 was ahead of unmet go-live conditions (blocklist category additions + validation list).
  • B-028 (Meridian crisis_gate): FIXED in code; unit tests pass (test_gate_fires_on_b028_roleplay_suicide_note, multi-turn drift). Still must be checked off on the validation doc with regression evidence before re-flip.
  • Device verify (earlier today): Section 2 UI on Build 84 was compliant when flag was true — good engineering proof; not a substitute for Mic’s full validation list.
  • Meridian m1: remains held (PHI hosting + Arc counsel).

Separate track

Mic open to paid clinical advisor role — commercial/ops only; does not change this clinical artifact.
