Ingest expectations (consumer)

• POST bearer auth, constant-time token compare
• Idempotent on run_id
• Reject unknown schema_version with 422

Producer obligations

1. Write telemetry.json into each archived report directory
2. Optionally POST to MERIDIAN_TELEMETRY_URL when env is set
3. Include all dispositions from reviews/ at run time
4. Never include credentials, tokens, or non-synthetic PHI
